1.  Select the Address tab.


2.  Select a Tenant. 


3.  Select the Disease/Diagnosis sub tab.


4.  Click on the New Disease/Diagnosis button.


5.  Click on the Magnifying Glass to the right of the Disease/Diagnosis Name field and a drop down list will display.  


6.  Select an item from the list.


7.  Date of Onset will automatically populate with the current date and can be changed manually if required.  


8.  Select a Disease/Diagnosis Status.


9.  In the Location box diagrams of both the front and back of the body will be displayed and the name of the Disease/Diagnosis will be visible in the top right corner of the the box. Click on the Disease name and drag it to the areas of the body that is affected.  


10.  Attach any applicable Notes or Documents.


11.  Select Save.
